The connected person: The broader applications of digital technology supporting customer-centric care
Using digital health technologies, the Connected Person has access to support and assistance at all times, from home or on the move. Through a suite of monitoring, communication and mobile assistive technologies, patients are free to live their lives and successfully manage long-term conditions, supported by their care team. Smart technologies enable clinicians to have a connected relationship with patients, and gather a more complete picture of a person and their healthcare needs. Through this holistic view, we can expand our support beyond chronic health conditions and offer tailored and patient-centred care that responds to individual needs.
