Personalised healthcare: Home monitoring at Barwon Health
This presentation will cover the small pilot programs aims at exploring the value of providing an at home monitoring service to support patient in the management of their chronic disease using a telehealth solution. The system utilises an individualied care plan that supports the patient self-management of their health care requirements.

Jo has been working in nursing leadership roles for over 15 years. The main focus of her career has been in the area of Rehabilitation Nursing with a move into service delivery in the Community just over 4 years ago. Significant accomplishments have been the establishment of the Information and Access Service providing a single point for all referrals for the diverse services on offer at Barwon Health community service. Currently Jo is leading the implementation of the Personalised Health Care Pilot for Barwon Health, working towards improving care delivery in the community through innovation.
Telehealth in residential aged care project - opening doors for the older person
This presentation will cover the background of the project, the scope of the project in the aged care environment, an example of its application, how has it worked, response from patients and clinicians, cost considerations and any barriers encountered. It will also address where to take it in the future?
Heather Cameron Project Officer Telehealth in Residential Aged Care, Aged Care, Barwon Health
Currently employed as the Project Officer for the Telehealth in Residential Aged Care Project at Barwon Health, Geelong, Victoria, Heather is leading the implementation of telehealth across 16 Residential Aged Care Facilities, housing 1400 residents, with the aim of improving residents' access to short term healthcare interventions, specifically medical specialists. Heather's background work spans over 25 years of healthcare across New Zealand and Australia. Her roles have included clinical areas of counselling and social work, education, project work, management and health promotion. As well, Heather has significant experience in teaching, training and professional writing. Her published work includes, "Different but the same; young people talk about living with serious illness" (Lothian, Melbourne, 1998).
