Understand how effective HealthPathways
are being developed and implemented through the collective strengths of
Medicare Locals, Local Health Districts and other local clinicians.
HealthPathways, a highly
successful model of collaboration first developed in New Zealand, has gained
traction with many Medicare Locals and Local Health Districts in Australia. The
name HealthPathways refers to local guidelines or ‘pathways’ used by General
Practitioners to link patients to the best treatment, local service or
specialist in a timely and effective way.
Although aimed for use in primary care, the pathways also provide value
to hospital specialists, practice nurses/managers, residential aged care
facilities and community allied health providers. Results have shown that the
use of HealthPathways improves communication between hospital specialists and
general practitioners and encourages the appropriate use of hospital resources.
Following the session
attendees will be able to:
- Articulate the concept of
HealthPathways, including the value and benefits they provide to both patients
and clinicians;
- Understand the governance
model adopted in the development and ongoing management of HealthPathways;
- Grasp the importance of
information relating to service gaps and health service utilisation and
availability in developing pathways within a local area; and
- Have a base understanding
of some of the technology used as core infrastructure to support HealthPathways.

Dr Angus Ritchie Renal Physician,
Clinical Lead, Health Pathways
Dr Angus Ritchie is a Sydney-based Renal Physician and Director
of Kidney School, the national education program for renal trainees. He is
currently the Clinical Lead for two major projects in Sydney Local Health
District: HealthPathways Sydney and Electronic Medication Management at Concord
Hospital. His quality improvement project “One Less Prick”, completed as part
of the Clinical Excellence Commission’s Executive Clinical Leadership Program,
lead to a 7% reduction in inpatient pathology testing at a tertiary teaching
hospital. He is interested in systems to improve patient care that focus on
value, data and innovation. He previously volunteered in Fiji as an Australian
Youth Ambassador for Development, working as a Medical School Lecturer and
running a haemodialysis unit. When he isn’t pottering in the garden or drinking
flat whites he’s trying to be a good husband to his wife Meg and a hands-on dad

Dr Linda Mann General Practitioner,
Clinical Lead, IWSML
Dr
Linda Mann is a GP who works in Sydney’s Inner West (in a practice which has
been providing care for patients under various names since 1987) and at Royal
Prince Alfred Hospital (where she is the VMO for the Young Parents Antenatal
Clinic). For the last six years Linda
has also given over one month per year to work in Borroloola in the Northern
Territory. She has undertaken various
roles on committees and as a leader, for Central Sydney GP Network, Inner West
Sydney Medicare Local and GP Synergy. Linda
now concentrates on one on one teaching of GP registrars, honing their
consultation skills, and influencing colleagues as the Clinical Lead for Health
Pathways Sydney. Linda has been an
editor for Diabetes Management in General Practice, a document which graces the
desks of every GP in Australia, over the last decade. In between, she plays
Bass Clarinet with Inner West Community Band.

Paul Campbell E-Health Project
Manager, WentWest
Paul is an experienced project
director having applied his skills in health ICT and construction projects, organisational restructuring, and passports and border security. Besides Australia, Paul has worked throughout
South East Asia, the Middle East and the USA. His most recent engagements have
been with The Salvation Army rebuilding their Residential Aged Care Facilities
in Queensland, the ACT and New South Wales; Engineering Infrastructure Manager
for an 1,800 home property development in the Camden area and; currently with
WentWest, the Western Sydney Medicare Local, as the Project Manager for the implementation
of e-health systems in Western Sydney and the Hills District. Paul has been President of the
NSW Chapter of the Australian Institute of Project Management (AIPM), Past
Board member of the Dubai Quality Group and is a current member of the Health
Informatics Society of Australia. He is
accredited under the AIPM certification as a Certified Practising Project
Director.
Jasmine Glennan HealthPathways Programme Manager, WentWest
Jasmine
Glennan is a Registered Nurse and has held roles within aged care,
primary care and acute facilities across Sydney. Jasmine
joined Western Sydney Medicare Local (Wentwest) in 2011 working across Mental Health and Community Health (HealthOne). Since inception in early
2013, Jasmine has led the development of HealthPathways in Western Sydney as
Programme Manager. Jasmine is passionate about improving the interface between
the health sectors, ultimately improving the journey of patients, and is
a proud advocate of HealthPathways.
