Using data to fill the gaps in primary health care

Health Informatics Society of Australia – South Australia
has developed an exciting calendar of events for 2013-14 to provide a practical approach to better understanding the quality,
access and power of the population health data collected by various
sectors.
It is intended that through this series we can explore
opportunities to share, collaborate and maybe even agree on its use in the
future to improve the planning and delivery of health care in South Australia.

General Practice SA
Within
a general practice setting, the patient data that we collect and store is
commonly used for providing individual care at the practice level. Attempts to access de-identified data for
population health, service delivery planning and data quality initiatives –
particularly from a third party - may be met with suspicion and resistance
where the clinician/s consider that their existing data collection, storage and
use are already ‘fit for purpose’? And
what is the value of that data – to the clinician, the practice, health
informaticians and health service planners?
General
Practice SA has a unique approach to implementing sustainable improvement in
clinical data quality at a practice level to improve and inform local, regional
and state primary health planning, funding and decision-making to deliver
quality health care solutions for patients.

Northern Adelaide Medicare Local
Presentation: Mapping the current health
landscape and the role of Medicare Locals
All Medicare Locals are
responsible for mapping local health care service providers and identifying
gaps in the health care sector within their catchment area and develop a
comprehensive regional needs assessment.
Northern Adelaide Medicare Local
(NAML) has been working with various stakeholders to collect data and has
commissioned a number of tools to assist in mapping social determinants of
health and demographics of the region. NAML has been working with various
stakeholders to identify gaps in services available in the northern Adelaide
region and has a strategy to partner and collaborate with providers to close
some of the gaps identified.
NAMLs approach is to work with
members and partners to use data to identify, map, collaborate, integrate and
assist in enhancing the patient journey through the primary health care system.
