Right treatment, right patient, right time: Transforming data into decisions

Patricia Correll will present an overview of the Lumos program in NSW, which securely links de-identified data from general practices (GPs) to other health data such as hospital, emergency department, mortality and more. It is a partnership between Primary
Health Networks and the NSW Ministry of Health to shine a light on patient journey by providing up-to date information about what services are used by patients, where and when. Lumos has been scaling up across NSW since late 2019 and currently has
over 500 NSW GPs enrolled which represent around 3 million people. Initial results already indicate that GP approaches to patient care can affect patient outcomes across the health system.
Jane Shrapnel will illustrate how Sydney Children’s Hospitals Network is using eMR data to create a data warehouse which underpins the analytics required to develop decision support for clinicians and optimise individual patient care. Jane will illustrate
how their research and algorithms have already improved patient outcomes for specific diseases, demonstrating the potential for population and cohort level precision medicine utilising eMR data.
