Digital Health Live | Pharmacie-R: The digital integration of a readmission risk tool

Pharmacie-R is a web-based, risk prediction tool which uses known risk variables to improve a clinician’s ability to identify complex patients in need of medication review services post-discharge. Experts from South Metropolitan Health Service and the Complex Needs Coordination Team (CoNeCT) join us to detail how this app can help predict risk of medication-related harm or readmission, especially in complex patients. Event price is $30 for non-members and free for all AIDH members.

Pharmacie-R: The digital integration of a readmission risk tool

In 2017 the World Health Organisation highlighted transitions of care and polypharmacy as priority areas in the 3rd global patient safety challenge and in 2019 the Australian government announced medication safety as its 10th National Health Priority. 

The Complex Needs Coordination Team (CoNeCT) provides an early post-discharge outreach medication management service for patients considered to be high-risk of medication-related harm.  These patients are identified by the hospital team - usually pharmacists, nurses or doctors. Research has shown clinicians cannot easily predict risk of harm or readmission, especially in complex patients. How can we see complexity in a hospital bed?

To address this, we have worked to develop an App, and a web-based platform to improve clinician’s ability to identify complex patients in need of medication review services post-discharge. This tool uses risk variables known to be clinically important causes of medication-related harm. To our knowledge, Pharmacie-R is the first risk prediction tool focused on polypharmacy and high-risk medicines which can be used immediately prior to discharge to target referrals to an outreach team.
